Meet Brooklyn! She’s part of Citizen Care’s Medically Complex Conditions Life Sharing program, supported by her amazing dad, Greg. Brooklyn had been waiting for a heart transplant since 2023 and received a transplant in February of 2026!

Brookyn and her family have turned their journey into something powerful.
In early 2025, she was named Erie’s Heart Hero and even got to attend the Erie Heart Ball, where she met Miss Pennsylvania!
In 2025, Brooklyn and her family helped promote the Erie Heart Walk, where Brooklyn proudly cut the ribbon to kick off the event.
She and her family walked as Brooklyn’s Transplant Team, spreading awareness and advocating for heart health every chance they get.
Thanks to Life Sharing, Brooklyn gets to live with her family, stay active in her community and advocate for herself, all while receiving the support she needs to thrive.
